Check your insurance provider’s website or call them directly to clarify what your in-network and out-of-network benefits are for your plan. Each plan has a different deductible and out-of-pocket amount. Some plans don’t offer out-of-network benefits, in which case you would pay out-of-pocket for treatment services that are not in your insurance plan’s network.
Depending on your insurance provider, you will see it clearly marked on the front or back of your insurance card whether you have a preferred provider organization (PPO), exclusive provider organization (EPO) or health maintenance organization (HMO). You will have different levels of co-pays, premiums, deductibles and in-network and out-of-pocket costs depending on which type of insurance coverage you have.
