Legal Notice: Discovery Practice Management, Inc. (“DPM”), a wholly owned subsidiary of Behavioral Health Holdings II, Inc. (“BHH II”), located at 18401 Von Karman, Suite 500, Irvine, CA 92612, is filing a Certificate of Need application pursuant to Section 19a-638 of the Connecticut General Statutes in connection with a forthcoming transaction where Unitranche Loan Transaction II, LLC or its affiliate will acquire either the assets (including the equity of DPM) or the equity of BHH II. DPM owns and operates the following facilities in the state of Connecticut: (i) Discovery Southport Adult OP located at 3530 Post Road Suite 304/306, Southport CT 06890; (ii) Center for Discovery, Greenwich located at 7 Riversville Rd., Ste 2, Greenwich CT 06831; (iii) Center for Discovery Residential Treatment for Adult Women with Eating Disorders located at 4536 Congress Street, Fairfield, CT 06824; and (iv) Center for Discovery Residential Treatment for Adult Women with Eating Disorders located at 600 Wellington Dr., Fairfield, CT 06824. The capital expenditures associated with the project are $2,766,000.
